Dr. Sean has gained expertise in correcting the appearance of many poorly performed hair transplants resulting from poor techniques, wrong, or outdated methods. Bad hair transplant techniques result in pluggy, or unnatural looking hairlines. Unfortunately for many who had bad hair transplants, the results were psychologically disastrous. The sad emotional aftermath of a bad hair transplant could be even worse than feeling of balding.
Dr. Sean Behnam has been a major contributor to new methods, including using Acell, for correcting the problems caused by outdated hair transplant procedures. The outdated procedures created many cosmetic problems. Unnatural hair lines, pluggy looking, poor hair direction, and inconsistent hair growths were most common.
What is scar revision?
Scar revision is a procedure that removes scars of hair transplant in the donor area. The end result is a minimal looking scar that can easily be concealed.
Why would somebody need scar revision?
Many older hair transplant techniques may be used in the process of strip hair transplantation may leave linear scar in the back of the scalp that are wide or hard to conceal. If somebody does have a scar from a previous hair transplant, Dr. Behnam can remove that scar and suture the area again in order to make it look better.
If the old scar has to be re-incised, how does this process reduce the appearance of the new scar to make it look it smaller, thinner, and less visible?
There are four factors involved. Typically, when a strip of tissue is removed for hair restoration, the width of the tissue is at least 1 cm. In scar revisions, the focus and goal is only to remove the scars, so there is less than 1 cm.
Dr. Behnam uses a triple method technique to reduce scarring.
The patient must do at least two weeks of scalp exercise. Scalp exercise increases the elasticity of the skin. It will reduce the tension on the back of the scalp when that strip of tissue is removed. Tension between the upper and lower skin edges is what leads to stretched scars.
Dr. Behnam uses a double layer closure during the procedure. This is when “inside of the wound and outsides are closed.” Dr. Behnam uses dissolvable suture for the inside of the wound and then closes the outside edges using strong staples. The inner sutures reduce tension and the outside staples prevent stretching of the scar. Dr. Behnam also uses the trichophytic closure technique. In this technique, a special cut is made such that the hairs grow into the level of the scar when it heal up and makes it look less visible.
How is scar revision performed?
For approximately 2 weeks prior to the procedure, you will be doing special scalp exercise that increases the laxity of skin. Then on the day of the procedure, the area is numbed up and the scar is removed. The inside of the wound is then closed with a dissolvable suture. The trichophytic technique is then used such that when the hair grows out, the new hair grows into the level of scar.
How often does Dr. Behnam do scar revision?

How often has Dr. Behnam had to revise his own scar?
Since implementing the triple method technique in 2008, we have not had any patient that required scar revision.
What is the importance of ACell in scar revision?
ACell is an extracellular matrix that promotes wound healing and decreases scarring. It is FDA approved for wound healing and provides a significant advantage in scar revision procedures.
What is trichophytic closure and its importance scar revision?
Trichophytic closure refers to a specific closure method such that the hair grafts grow into the scar. During the actual procedure, special cut is made. A few months after when the wound has healed, the hair should actually grow at the level of the scar and camouflage it. Dr. Behnam uses the trichophytic closure on every patient.
What is double-layer closure and its importance in scar revision?
Double-layer closure refers to the special closure of the wound. When a strip is removed, the inner layer is sutured completely, then the outer edges are also closed up. This double closure technique leads to minimal scarring.
Will the scar be completely gone after scar revision?
No, there will still be very light scar visible, however it will be minimized, and easily camouflaged.
Instead of removing a scar, can a graft be placed into the scar?
Yes, this is known as a scar camouflage with grafts. Typically follicular unit extractions are used o remove the individual grafts which are then placed into the level of the scars.
How is scar camouflage different than scar revision?
In scar revision, the actual scar is cut and removed and in scar camouflage, individual grafts are placed into level of the scar to camouflage it.
What is the cost of scar revision?
The cost of scar revision is $750. The use of ACell is $750 as well.
What if some hairs are also removed along with the scar, what will happen those hairs? Will I have to pay for them to placed?

Four Phases of Healing
Typically, absent the use of ACELL Extracellular Matrix Technology, there are four phases to the process of healing a wound:
- The Inflammatory Phase involves bleeding, narrowing of blood vessels, clot formation, and release of chemical substances that will begin healing.
- Next is the Proliferative Phase in which a matrix or latticework of cells begins to form scar tissue.
- The Remodeling Phase begins after 2-3 weeks when the matrix becomes more organized, strengthening the scar tissue.
- The Epithelialization Phase is the process of laying down new skin covering the scar tissue.
Efforts to improve wound care treatment have focused on developing anti-inflammatory drugs and improving surgical techniques, neither of which address the disfiguration and loss of function associated with scarring.

| I had a hair transplant done by Dr Sean in 2010. I never write a review. I am very please. I was completely bald. I had a hair transplant in 2007 by another doctor, but it didnt take so well. I was very skeptical. I was left with big plugs in the front of the scalp. Then, I was referred to Dr Behnam by a friend of mine. I went to see him. I was surprised that he spent 2 hours planning, desining. I was convinced that this is the doctor to fix up my previous blunder. So in Jan 2010, I had a hair transplant. The day went very well, almost painless. The difference was amazing from the begining. It has now been 1 year 3 months, the hair has grown out. Dr Shawn Benham has fixed the plugs. The hair translplant looks very natural. I can go out again. I have regained my social life. The doctor makes such a huge difference. I have already reccommended Dr Behnam to a lot of my friends and consistently have the same results. Highly recommended. |

| I never really considered myself a transplant candidate before now. Anyway, had my procedure with Dr. Behnam yesterday and everything went well. No pain whatsoever and his staff and technicians were professional and courteous, answering all my questions while working efficiently. My only disappointment was my graft count, which was less than expected (due to low density in donor area). After the procedure was completed the doctor explained the lower than expected graft count and even refunded part of the cost back to me. Oh well, I would have preferred the grafts instead, but he could only work with what he had. I would highly recommend Dr. B to anyone considering this type of procedure. |

Laser Resurfacing For Hair Transplant Scars
